* Key Responsibilities:

****** Patient Care & Coordination:
*** Provide direct patient care, including assessments and medical procedures, under RN supervision.
* Follow physician orders and the nursing process to manage patients’ total needs.
* Coordinate patient care with the RN, therapists, and other team members, ensuring continuity of care.
* Report changes in patients’ conditions and contribute to care plan adjustments.
* Meet established productivity standards and deliver patient-centered care.
** Patient & Caregiver

Education:

*** Educate patients and caregivers on self-care, promoting independence and safety.
* Actively monitor and report any safety concerns to appropriate authorities or leadership.
** Communication & Documentation:
*** Complete accurate and timely documentation according to regulatory guidelines.
* Actively participate in care team meetings and contribute to collaborative problem-solving.
** Staff Education & Professional Growth:
*** Stay current with clinical knowledge by attending continuing education, in-services, and staff development activities.
** Additional Responsibilities:
*** Participate in special projects and assist with other duties as assigned by leadership.
* Comply with all Wellstar Health System policies, procedures, and standards of care.

* Required Qualifications:

****** Education**:
* ** High School Diploma or equivalent required.
**** Licensure**:
* ** Valid LPN license in Georgia required.
**** Experience**:
* ** Minimum of 3 years of medical/surgical nursing experience in an acute care setting is preferred.

* Certifications:

**
* ** Basic Life Support (BLS) certification from AHA required.
**** Skills & Abilities:**
* ** Strong clinical assessment skills and attention to detail.**
* ** Excellent organizational, time-management, and problem-solving abilities.**
* ** Exceptional written and verbal communication skills.
