Medical Case Manager

Essential Functions (Duties and Responsibilities):

Perform other related duties as assigned by Supervisor or Executive Director.

Knowledge, Skills and Abilities:

Basic knowledge of HIV/AIDS Ability and willingness to use both knowledge and personal experience in facilitating linkage of individuals and groups to essential HIV health related services

Communication skills and proven behaviors consistent with maintenance of health and wellness. Ability to establish trust based relationships with people in an appropriate and ethical manner

Knowledge and ability to access community resources Ability to attend mandatory staff meetings Basic computer skills and ability to collect data elements and generate reports in a timely manner

Education and Experience:

High school diploma with commensurate experience; associate or bachelor degree welcome

Familiarity with people living with HIV is mandatory

Familiarity with Nashville/Davidson and/or surrounding Counties Familiarity with Ryan White programs and Meharry Community Wellness Center is highly desirable#
