Base pay range
$55.00/hr - $60.00/hr
Duties
- Assess patients, coordinate care, and plan discharges.
- Organize and participate in family meetings.
- Contribute to the development of treatment plans.
- Arrange DME, SNF, and ambulance transport independently.
- Utilize computerized documentation systems.
- Work with HMOs, IPAs, and understand adoption/foster care, drug/alcohol abuse treatment, mental health crises, and protective services.
