Overview
Description The Inpatient Licensed Clinical Social Worker (LCSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Responsibilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management LCSW is to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive/behavioral or legal barriers to accessing necessary resources and care.
Responsibilities- Provide professional, comprehensive care management services for at risk patients in an acute care environment.
- Conduct assessment and planning, coordinate care, and discharge planning.
- Document interventions and ensure regulatory compliance and patient advocacy.
- Identify and address barriers to accessing necessary resources and care (social, financial, cognitive/behavioral or legal).
- Aim to ensure continuity of care and patient-centric outcomes for vulnerable populations.
- Master s Degree in Social Work from an Accredited Council on Social Work Education program
- California Clinical Social Worker License (upon hire)
- 2 years of social work experience in a healthcare setting
- 1 year of experience in an acute care setting or successful completion of TIPs program or Case Management Orientation Program
- Basic understanding of acute medical-surgical terminology
- Prior case management experience
