Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively with the interprofessional care team, the Registered Nurse/Registered Psychiatric Nurse provides and coordinates the delivery of nursing care services to clients experiencing problematic substance use. This includes assessment, referrals, counselling, client and family teaching, and follow-up as required, along with health promotion activities.

Demonstrates nursing expertise and provides clinical leadership in evidence-based practice through direct care activities including nursing assessments, crisis intervention, treatment and group facilitation.

Responsibilities
  • Plan and provide direct nursing care to clients in compliance with standards of practice; review client/family history, conduct physical/psychosocial assessments, develop client-specific care plans, and collaborate with the health care team.
  • Process physician or nurse practitioner orders, initiate unit standing orders, administer medications, carry out treatments, and monitor/document effects.
  • Manage emergencies (e.g., drug overdose, heart attack, assaultive behaviour, seizures) in accordance with policies and procedures (e.g., CPR, ambulance notification).
  • Consult and collaborate with interdisciplinary team members; participate in client care conferences, rounds, and staff meetings; contribute information for care planning.
  • Provide general and crisis counselling to clients presenting to Emergency Department, IPU, and medical wards; teach clients and families about illness prevention, health maintenance, and health promotion; assess learning needs and evaluate outcomes.
  • As assigned, act within the RN/RPN scope of practice, identifies when client needs exceed scope, and communicate accordingly.
  • Participate in quality improvement and risk management activities; identify high-risk situations; evaluate changes in nursing practice; contribute to safety audits and protocol updates.
  • Serve as a clinical resource and provide nursing staff training, mentoring, and role modelling; maintain clinical records accurately (e.g., assessments, progress notes, interventions, outcomes, discharge summaries).
  • Perform intake functions, conduct mental health assessments, develop and adjust comprehensive treatment plans, administer prescribed medications, and monitor mental state and response to treatment.
  • Maintain a safe environment by following safety and emergency procedures and reporting risks to the Client Care Coordinator.
  • Participate in program development and delivery of clinical services; contribute to development, implementation, and evaluation of client program activities and treatment approaches.
  • Identify personal learning needs and pursue development with the Client Care Coordinator; assist in policy/procedure development and interpretation for clients and other agencies.
  • If applicable, plan and deliver educational sessions to clients, family members, other service providers, and community agencies; facilitate group sessions using relevant therapeutic approaches in collaboration with the interdisciplinary team.
