As an RN Case Manager
, you’ll provide skilled nursing care to hospice patients in their place of residence, coordinate care with the interdisciplinary team, and support patients and families through every stage of the hospice journey. This role is ideal for nurses who value independence, collaboration, and purpose-driven work.
- Complete initial and ongoing nursing assessments and reassessments.
- Develop, implement, and update individualized plans of care.
- Coordinate care with physicians, hospice team members, patients, and families.
- Provide skilled nursing services and patient/family education.
- Monitor assigned cases to ensure regulatory and payer compliance.
- Complete clinical documentation accurately and timely.
- Supervise and provide direction to LPNs, aides, and other support staff as applicable.
- Participate in on-call rotation as assigned.
Required:
- Graduate of an accredited School of Nursing (Diploma, Associate, or Bachelor’s).
- Current Registered Nurse (RN) license in the state of Indiana.
- Valid driver’s license, reliable transportation, and auto insurance.
- Current CPR certification.
- At least one year of RN experience in a clinical setting.
- Hospice or home health experience.
- Strong communication, organization, and critical-thinking skills.
