Job Description for Case Manager RN
  • Conducts initial and ongoing comprehensive assessments of the patient and family's physical, psychosocial, environmental, safety and spiritual needs within 5 days of entry to hospice services, with reassessments no less frequently than every 15 days.
  • Develops an individualized plan of care based on the comprehensive assessment and the patient’s and family's goals.
  • Assesses pain and comfort, developing and implementing an individualized pain management plan.
  • Provides holistic, patient/family‑centered care at the patient’s residence to improve quality of life.
  • Consults and collaborates with the interdisciplinary team and other agencies involved in the patient’s care.
  • Supports the patient, family and caregiver in medical decision‑making and unique spiritual and cultural beliefs.
  • Provides supervision of LPN and Home Health/Hospice aides in accordance with agency policy and regulatory requirements.
  • Performs admissions as assigned.
  • Completes an average of 25 visits per week.
  • Communicates appropriate information and participates at IDG meetings.
  • Documents problems, nursing assessments, appropriate goals, care provided, interventions, patient/family teaching, and outcomes.
  • Completes patient assessment documentation on the day care is given.
  • Documents all physician orders, IDG notes, and patient notes on the day work is performed.
  • Documents LPN, Home Health, and Hospice Aide supervisory visits every 14 days.
  • Ensures verbal and written reports are clear, concise, and timely to provide continuity of care.
  • Teaches patient self‑care.
  • Instructs patients and families (or ECF staff) on appropriate aspects of patient care.
  • Provides medication education: indications, side effects, contraindications.
  • Assists patients’ families (or ECF staff) in understanding illness, the dying process, plan of care, and patient needs.
  • Educates patients and families on hospice care and benefits rights.
  • Serves as preceptor to other staff nurses, aides, and students.
  • Committed to agency resolution against fraud and abuse.
  • Knowledgeable in hospice philosophy, LCDs, and state and federal rules and regulations.
  • Arrives to work and meetings on time and ready to work.
  • Respects confidentiality of patients/families and other agency employees.
  • Attends monthly nurse meetings and educational conferences for professional growth as required.
  • Attends and completes yearly nursing skills validations.
  • Performs other duties that may be assigned.
Success Capabilities
  • Licensed RN in the State of Indiana
  • Current CPR certification
  • Valid driver’s license, good driving record and auto accident insurance
Conditions of Employment
  • Must pass drug screen and background check.
  • Pass TB test.
  • Provide an active auto insurance policy as a licensed driver in Indiana with the required coverage.
  • Provide an active driver’s license.
  • Provide proof of eligibility to work in the United States.
  • Maintain a high level of integrity, ethics and professionalism.
