- Position Summary:
The RN, Rehab provides comprehensive nursing care to patients recovering from illness, injury, or surgery, supporting functional improvement and a safe return to daily activities. This role conducts detailed assessments, monitors patient progress, and delivers evidence-based nursing interventions that enhance mobility, strength, endurance, and self-care. - Working collaboratively with physical, occupational, and speech therapy teams, the RN helps coordinate rehabilitation plans and reinforce therapeutic goals throughout the patient’s stay.
- As part of a supportive, interdisciplinary rehabilitation environment, this position promotes patient and family understanding of treatment expectations, home safety, and long-term health management through clear education and ongoing communication.
- Perform ongoing assessments of patient status, functional abilities, and rehabilitation progress, responding promptly to changes in condition.
- Coordinate individualized care plans with therapy disciplines and physicians to support goal-directed rehabilitation and safe transitions of care.
- Administer medications, treatments, and therapeutic interventions while monitoring their effectiveness and patient tolerance.
- Maintain accurate and timely documentation of assessments, interventions, functional outcomes, and patient education within the electronic health record.
- Educate patients and families on self-care strategies, mobility techniques, equipment use, and discharge instructions to promote independence and safety.
- Collaborate with interdisciplinary team members to enhance patient engagement, reduce complications, and support quality and safety initiatives in the rehab setting.
- Licenses and Certifications
Required:
Current or temporary RN license in North Dakota or compact RN licensure. BLS certification required within orientation period. Specialty certification in area of practice is desirable. - Educational Requirements: RN program as evidenced by licensure. Official diploma or transcript from the school/university where the Degree was received.
- Experience Requirements: Hospital experience is preferred, but not required.
- Special Skills or Training Requirements: As required by specific units.
- Physical Requirements: The RN, Rehab frequently stands, walks, and assists patients with mobility activities, including transfers, gait training support, and repositioning. The role requires pushing and pulling equipment, assisting with adaptive devices, and performing moderate lifting with proper body mechanics and team support. Adequate vision, hearing, manual dexterity, and communication abilities are essential for monitoring patient conditions, safely guiding mobility efforts, and performing clinical procedures.
- Environmental Requirements: This position functions in an inpatient rehabilitation environment with regular exposure to infectious diseases, bodily fluids, cleaning agents, and rehabilitation equipment. The workload may involve variable noise levels, patient movement, and periods of high activity requiring sustained attention and adherence to safety practices. Proper use of PPE, infection-prevention procedures, and compliance with Trinity Health safety and equipment-handling protocols are essential to maintaining a safe environment for patients and staff.
