Minot, ND
- Schedule:
40 hrs/week (5x8 days)

* Key Requirements:
** - Licensed Physical Therapist in North Dakota - Minimum 2 years of experience as a Physical Therapist preferred - Current driver's license and car available for patient home travel - Recent travel experience required - Cerner experience strongly preferred - Locals not accepted – 50 mile radius rule
** Duties:** - Provide rehabilitative services to home health and hospice patients - Contribute to treatment, rehabilitation, and safety for home health patients - Assist in maintenance, education, and safety for hospice patients and families - Coordinate therapy services for individuals and families in care - Assess individual and family health needs and potential hazards - Consult and refer families to appropriate personnel within the agency or community services - Carry out treatment and preventative procedures under medical direction - Work under supervision to evaluate, plan, implement, supervise, and document patient care, focusing on the geriatric population
