North Central Behavioral Health Clinic - Minot, ND - Case Manager - Youth & Family Team

Be part of the solution to the behavioral health crisis. As a team member for ND Department of Health & Human Services, you will ensure the most vulnerable have access to a high-quality healthcare system in which prevention works, treatment is effective, and people recover. As a Case Manager, you will provide individuals opportunities to manage their own recovery so they can live and work independently in their community.

With the support of an interdisciplinary team, you will conduct comprehensive needs assessments, complete, and implement service plans, as well as leverage contacts and referral partners to ensure access to needed services. Case managers also assist with crisis response. Case Management services are community-based. Work will be done primarily in the individual's home and community.

To thrive in this position, you should be knowledgeable about resources offered for youth and families in your community and prefer working with the youth in their home and natural environments.

• Conduct comprehensive needs assessments, develop and implement service plans, and coordinate referrals to ensure access to needed services.
• Provide case management services in community settings, including the individual’s home, and assist with crisis response as needed.
• Collaborate with an interdisciplinary team to support the individual’s recovery and independent living in the community.

• One of the following: a bachelor’s degree or a master’s degree; OR at least five years of paid supervised experience working with individuals with SMI/SED in case management functions such as individual assessment, care plan development and maintenance, referral and appointment scheduling, monitoring and follow-up activities.

This position requires a valid driver’s license and successful completion of the assigned defensive driving course during new-hire orientation.
