Oversees Resident Centered Care.

Position Summary

This is a full-time, salaried leadership RN Director of Health Services nursing position. This schedule includes:

  • 80 hours per pay period
  • Triage Line (Minimal on‑call)

The DHS Nurse provides leadership in all aspects of nursing services while ensuring regulatory compliance and service excellence. Responsible for ensuring an RN is available 24/7; dedicated triage nurse supports evenings and overnight calls. Supports the Assisted Living staff and serves as manager on duty as needed.

Responsibilities
  • Ensures the development of RN assessments and implantation of individual resident care plans/service agreements that attain or maintain the highest level of physical, mental, and psychosocial well‑being, incorporating Honoring Choices (POLST) and Resident Bill of Rights.
  • Ensures appropriate resident/family participation and education in the care planning/service agreement processes, and documents properly.
Ensures Clinical Compliance
  • Plans, organizes, directs, and implements comprehensive health care program, including staff placement, medication administration program, resident safety, and staff development.
  • Collaborates with Ebenezer administration to develop nursing policies and procedures that reflect current clinical practice, professional standards, and Ebenezer’s philosophy of care.
  • Evaluates and modifies goals, objectives, and organizational structure of the health services department to continually meet best practices.
  • Oversees site compliance with WI Board of Nursing, Wisconsin State Home Care Licensure, HIPAA, OSHA, CLIA, and Medicaid.
  • Advises administration on nursing services and environmental safety, enlisting compliance specialists as needed.
  • Develops and participates in the implementation of a process that collects and analyzes health services data, identifies trends, institutes actions to resolve problems promptly, and evaluates effectiveness.
Manages Nursing Department Budget
  • Participates in budget development and monitors budget compliance.
  • Prepares financial and statistical reports as requested.
Provides Leadership and General Oversight of the Nursing Department
  • Develops goals with nursing staff and holds them accountable to job responsibilities.
  • Provides ongoing support and encourages professional development of the leadership team.
  • Establishes and monitors an effective staffing model, ensuring efficient use of staffing resources.
  • Oversees delivery of all services provided by care providers and ensures staff compliance with relevant regulations and Ebenezer policies.
  • Establishes ongoing training programs for nursing staff, consistent with local, state, and federal regulations.
  • Monitors employee satisfaction and leads development of employee engagement strategies.
  • Ensures 24‑hour availability of RN on‑call.
  • Performs essential functions of any health care team member if required.
Leads and/or Performs Coordination of Resident Care
  • Ensures availability of adequate tools to implement resident care plans, including supplies such as medications and durable medical equipment.
  • Encourages collaboration with community resources to meet resident identified needs.
  • Ensures accurate and timely communication of resident care plan needs with all internal departments such as dietary, billing, etc.
  • Ensures collaboration among resident, family, physicians, consultants, community agencies, and institutions to facilitate safe transfer of care as needed.
  • Ensures assessments are completed timely and MD orders updated to validate care plans.
