• Provide advanced nursing care to adult patients with acute, chronic, and complex medical conditions.
• Perform assessments, diagnoses, develop and implement treatment plans, and manage patient care.
• Conduct thorough patient histories and physical examinations.
• Diagnose and manage chronic illnesses such as hypertension, diabetes, COPD, heart disease, and other internal medicine conditions.
• Order, interpret, and act on diagnostic tests including labs, imaging, and screenings.
• Develop and implement individualized treatment plans, including prescribing medications and therapies in accordance with state laws and organizational policies.
• Provide patient education on disease prevention, health promotion, and management of chronic conditions.
• Monitor and evaluate patient progress and response to treatment, adjusting plans as necessary.
• Collaborate with physicians, specialists, and other healthcare providers to coordinate care.
• Document all patient encounters, treatments, and progress notes accurately in the electronic health record (EHR).
• Participate in quality improvement initiatives and stay current with evidence‑based practices in internal medicine.
• Maintain compliance with regulatory and organizational standards for patient care.

• Master’s or Doctorate degree from an accredited Nurse Practitioner program.
• If APRN was licensed prior to August 1, 1995, a master’s degree in nursing is preferred.
• Experience in internal medicine or related adult primary care setting preferred.
• Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice.
• Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances in each state of practice.
• State Controlled Substance License (if required by State).
• National board certification as a Family (FNP‑BC) or FNP‑C, Adult‑Gerontology Primary Care (AGPCNP‑BC) or AG‑NP, Adult (ANP‑C). Additional certifications require approval from a market executive leader:
  Adult‑Gerontology Acute Care (AGACNP‑BC) or ACNPC‑AG), Acute Care Adult (ACNP‑BC).
• Obtains and subsequently maintains required specific competencies and certifications as outlined on privilege form.
• Privileging documents and related credentialing requirements will supersede this job description in defining minimum qualifications, criteria, and scope of practice, if variation between documents exists.
