LPN - Licensed Practical Nurse

North Star Manor is a Long Term Care facility located in Warren, MN.

We are looking for compassionate, caring team members with the goal to serve and support our residents.

$30.00 - $36.50 / hour

Pay is based on years of applicable experience

$5000 Sign-On Bonus for full time positions; $2500 for part-time

$2/hour Shift Differential pay between 4pm and 6am

Provide resident care within established policies, procedures, and professional standards taking into with consideration and respect.

Administer medications and treatments, reporting changes in resident’s condition and takes necessary actions.

Notify family members, physicians, and other appropriate individuals when there are changes in resident’s condition or physician orders.

Complete various resident clinical records and forms including the care plan, daily resident charting, and medication administration record, lab results, etc.

Actively supervise Certified Nursing Assistants (CNA) and Certified Medication Assistants (CMA) ensuring staff are well informed of responsibilities.

Determine appropriate CNA assignments based on resident assessment, acuity, dx, and other care needs.

Current LPN licensure in the state of Minnesota is required

Current CPR certification or ability to become certified in CPR within 180 days of hire.
