* Benefit eligibility is dependent on employment status. Newly hired Variable Hour and Part Time employees will be eligible for ACA Benefits after the first twelve months of employment, provided they meet the required hours.
Qualifications of the Licensed Practical Nurse (LPN)- Valid Minnesota license (LPN) in good standing
- Current CPR certification or ability to obtain prior to providing care
- Provide one‑to‑one patient care in a home environment
- Execute physicians’ prescribed plans of care and maintain compliant documentation in the system of record
- Administer prescribed medication, treatments, and therapies
- Conduct patient assessments
- Coordinate care among providers
- Promote health, teach, and train family members
