Job Summary And Responsibilities

LPN Long Term Care PRN at CHI Lake Wood Health. As an LPN, you will help patients by providing nursing care to our Long‑Term Care (LTC) residents under the direction of a physician or registered nurse. Every day you will work with residents to evaluate their needs and provide physical or psychosocial care. You will demonstrate compassion, patience, and strong communication skills, and you will be organized and capable of prioritizing tasks independently while demonstrating leadership to resolve conflicts and achieve positive outcomes.

• Administers prescribed medications and treatments.
• Completes all required documentation in a timely manner.
• Administers, conducts, directs and supervises resident‑related care and activities on the assigned unit.

• Graduate of an accredited school of nursing.
• Minnesota (MN) Licensed Practical Nurse (LPN).
• Basic Life Support (BLS) certification.
