Job Title:Resource Nurse (Hospice) — Admissions, Case Management, Visit Nurse
Pay:$90,000 – $130,000 DOE
Benefits:Full Benefits Package
Schedule:Monday – Friday plus on-call and weekend requirements.
Location:Serving the Greater Twin Cities Metro Area (21-County Service Area)

What You’ll Do
- Admissions:Complete hospice eligibility assessments, consents/education, med reconciliation, immediate symptom plan, and safe‑start services.
- Case Management (as assigned):Own a patient panel—plan of care, symptom management, caregiver coaching, IDT coordination, timely documentation.
- Visit Nurse/Crisis Support:Provide PRN, routine, and urgent visits; execute standing orders; expedite collaboration with the MD/NP.
- Interdisciplinary
Collaboration:
Partner with social work, chaplaincy, aides, and therapy to support patient and family goals. - Documentation & Compliance:Chart accurately in the EMR; follow Medicare CoPs and agency policies.
What You’ll Bring
- Required hospice background
from one of the following: prior hospice agency experience or high‑acuity settings such as ICU, palliative care, or oncology. - Strength in admissions interviewing, symptom triage, and family education.
- Organized, decisive, and comfortable driving across the service area (reliable vehicle/insurance).
- Professional, compassionate communicator who thrives in a dynamic environment.
