- LPN: $28–$35 per hour
This position will work in collaboration with triage nursing to make skilled nursing visits to patients/families needing after‑hours / overnight support. Friday, Saturday, and Sunday shifts available.

- Graduate of an accredited school of nursing.
- Current state of Minnesota RN or LPN licensure.
- Three years recent nursing experience in acute, long‑term care, or home care/hospice/public health.
- Patient care planning and patient teaching skills.
- Written and verbal communication skills.
- Critical thinker.
- Current Minnesota driver’s license, insurance, and daily access to a reliable vehicle.
- Computer literacy.
- Displays an attitude of courtesy and respect for all prospective patients, families, staff, and referral sources.
- Works closely with triage nurse and makes face‑to‑face visits as directed.
- Functions as a team member with other staff in the performance of duties.
- Communicates significant findings, problems, patients’ needs, changes in condition or environment to the physician, clinical manager, and other members of the hospice care team and obtains physician orders as required.
- Seeks and accepts feedback regarding the performance of job duties.
- Is aware of patient confidentiality and adheres to HIPAA regulations.
- Displays an understanding of vulnerable adult issues.
- Displays an understanding of hospice patients’ rights.
- Exhibits sound mature judgment.
- Excellent patient care delivery.
