This nursing support position is eligible for a $1,500 sign‑on bonus!

Shift differentials of $1.00 evenings and $1.50 nights.

Ebenezer is seeking a Nursing Assistant/Registered (CNA/NAR) to support our adult residents in our Skilled Nursing Facility in Lake City, MN. This role provides hands‑on care and support services for residents in our Care Center long‑term care unit and young‑adult geriatric Transitional Care Unit patients.

This patient support schedule includes:

• 56 hours in two weeks
• Day shift
• Every other weekend and holiday rotation
• Reimbursement of previous costs to take a CNA course/state NA exams if this is your first time requesting reimbursement

Lake City Care Center Careers

• Assist residents based on individual care plans with personal cares such as bathing, oral hygiene, grooming, toileting, dressing, showers and responding to call lights.
• Check and record vitals per community patient care model.
• AM scanned / PM cares including peri care and catheter care.
• Turn, lift, or transfer residents with or without other persons/mechanical assistance.
• Observe residents and report status to appropriate staff in a clear and concise manner.
• Chart or document in electronic medical record system.
• Seek to resolve resident and family concerns.
• Receive and give report to appropriate staff during various times in work shift.
• Other duties such as ordering and maintaining cleanliness in rooms and work areas, reporting malfunctioning equipment to maintenance, and answering phones in a timely manner.

• Active on MN Nursing Assistant registry by start date.
• If hired into the Ebenezer Nursing Assistant Training program, granted a 4‑month grace period from hire date to be active on the MN Nursing Assistant registry.

• Ability to work with a diverse and geriatric population.
• Communicate efficiently, with empathy and understanding for residents and coworkers.
• Experience working as a HHA, PCA, DSP, NA, or Resident Assistant.
• Work experience in a customer service setting.

The posted compensation range corresponds to the minimum and maximum pay rates outlined in your union agreement for the respective job classifications at the time of job posting. An individual's pay rate within this range may be influenced by various factors, including FTE, skills, knowledge, educational background, and qualifying experience as specified in the contract.
