We are seeking an experienced Travel Physical Therapist (PT) for a 14-week Skilled Nursing / Long-Term Care (SNF/LTC) assignment in Minnesota . This position is covering a vacancy and offers a weekday day-shift schedule with a minimum guaranteed workload and standard SNF productivity expectations .

Assignment Details

• Location: Minnesota

• Contract Length: 14 weeks

• Schedule: Monday–Friday

• Hours: 8:00 AM – 5:00 PM

• Shift: Day shift

• Minimum Hours: 36 hours per week

• Productivity Expectation: 85%

Responsibilities

• Evaluate and treat residents in a skilled nursing / long-term care setting

• Develop and implement individualized physical therapy treatment plans

• Document evaluations, progress notes, and outcomes accurately and timely

• Collaborate with interdisciplinary rehab and nursing teams

• Educate patients and caregivers on mobility, safety, and functional goals

• Ensure compliance with clinical standards and regulatory requirements

Requirements

• Active Physical Therapist (PT) license

• SNF/LTC experience preferred

• Ability to meet productivity standards in a rehab setting

• Must meet all compliance and credentialing requirements

• Strong communication, organization, and clinical skills

This travel assignment is ideal for PTs seeking a stable weekday schedule , consistent caseload , and experience in a Skilled Nursing / Long-Term Care environment .
