As Director of Social Services
, your roleis to plan, develop, organize, implement, evaluate, and direct social service programs at the facility in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that medically related emotional and social needs of the resident are met/maintained on an individual basis. May have direct reports such as Social Services Assistants.

Other duties include:

• Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to leadership partners.
• Review department policies and procedures, at least annually, and participate in making recommended changes.
• Assist in developing and implementing policies and procedures for the identification of medically related social and emotional needs of the resident.
• Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
• Participate in discharge planning, development, and implementation of social care plans and resident assessments.
• Demonstrate compassion and understanding when working with families and residents during and other interactions to obtain social history and background information and while planning individualized social service programs when possible.

Must be a Registered Nurse with at least 3 years of supervisory experience.
