Position SummaryThe Clinical Dietitian provides nutrition services to residents in a long‑term care setting and works closely with the multidisciplinary healthcare team to deliver patient‑focused care that aligns with medical goals. Core duties include applying the Nutrition Care Process (NCP) to assess, diagnose, plan intervention, and monitor patient progress using the Scope of Dietetics Practice Framework (SODPF) and evidence‑based practice guidelines.
Key Responsibilities- Apply the Nutrition Care Process to geriatric patients.
- Conduct comprehensive nutrition assessments, ensuring MDS forms are completed and documented.
- Perform nutrition diagnosis and document findings in PES or PE format.
- Develop and implement intervention plans based on evidence‑based guidelines.
- Provide education and counseling to residents and families, and monitor progress.
- Collaborate with culinary and interdisciplinary teams; gather data and adjust plans as needed.
- Serve as a resource on nutrition issues, assist dietetics students, and participate in discharge planning.
- Ensure facility compliance with HIPAA, state licensing, and accrediting agency standards.
- Knowledge of the NCP and geriatric nutrition therapy.
- Critical thinking, problem‑solving, and collaboration.
- Excellent counseling and communication skills, effective with diverse communities.
- Proficiency in nutrient analysis, word processing, and spreadsheet software.
- Independently manage patient care units or outpatient clinics.
- Bachelor’s degree in dietetics, foods and nutrition, or related field from a U.S. regionally accredited institution.
- Completion of a CADE‑approved didactic program and supervised practice program.
- Registration eligible and must obtain license within 1 year.
- Qualify for employment after a criminal background check per Minnesota Department of Human Services guidelines.
- 1–2 years of experience in a healthcare or skilled nursing facility.
