Since 2016, Twin Cities Physicians has redefined how primary care is delivered across Minnesota's senior living communities. With over 4,500 residents under our care, we provide on-site, consistent medical services to improve outcomes and reduce hospitalizations.
We’re hiring a Physician Assistant (PA-C) who enjoys being on the move and working in non-traditional clinical settings. You'll serve residents in assisted living and memory care communities, building relationships and delivering meaningful, autonomy-driven care in place.
Responsibilities- Travel to multiple senior living communities weekly to provide medical care
- Conduct assessments, manage chronic conditions, and address acute concerns
- Partner with interdisciplinary teams to align on patient goals
- Maintain accurate, timely EMR documentation and communicate care plans
- Active MN PA license (Board of Medical Practice)
- Background in geriatrics, internal medicine, or complex care preferred
- Strong communication and time management skills
- Must have reliable transportation and current auto insurance

- Air Force: 42G1 – Physician Assistant
- Marines: – Physician Assistant
- Coast Guard: – Physician Assistant
