- Educate patients and caregivers on home exercise routines and fall prevention strategies, empowering them with confidence
- Collaborate regularly with the supervising PT and interdisciplinary care team to ensure holistic patient care
- Licensed Physical Therapy Assistant in the state of Minnesota
- Valid driver’s license
, reliable vehicle, and current insurance - Strong communication skills
, with the ability to educate and motivate patients - Home care experience is a plus— but we will train the right individual passionate about making a difference
