Position: Case Manager II - Waiver Case Management (Hennepin County)

Case Manager II

People Incorporated is seeking a Case Manager II to join our Waiver Case Management Hennepin County team to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.

Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. We value work-life balance and have manageable caseloads that are around 30 clients on average!

Hours: Full-Time, Monday-Friday, 8am-4pm with flexible business hours

Location: Hennepin County, Minnesota

Range: $24.25 - $25.75/hourly based on qualifications

Hiring Bonus: $1,000 to external candidates

Responsibilities
  • Must be able to have onsite presence within the Twin Cities, Minnesota.
  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.
