As a Resettlement Case Manager, you will assist newly arrived refugee and immigrant families who are brand new to the United States and need significant connections to community services and enrollment into essential community programs. Case managers establish a self-sufficiency plan with each individual/family and work toward stability with each objective over the three-month intensive resettlement period as dictated by the US State Department regulations.
Case management includes coordinating housing, medical, education, employment, and health services as well as other means of supporting the individual(s) to achieve self-sufficiency and wellbeing.
- Assist newly arrived refugee and immigrant families with connections to community services and enrollment into essential programs
- Establish a self-sufficiency plan with each individual/family and support stability over the three-month intensive resettlement period
- Coordinate housing, medical, education, employment, and health services to support self-sufficiency and wellbeing
- Bachelor's degree in social work, psychology, human services, political science and/or experience working in human services or social work field
- Six to twelve months of experience providing social services to refugees or immigrants (preferred)
- Experience completing case notes and county applications for MFIP, MNsure, and SNAP
- MS Office skills required (Word, Excel, Outlook)
- Ability to work with children
- Ability to drive families, including children who require use of a car seat, in personal and agency vehicles
- Valid driver's license, acceptable driving record, access to reliable transportation, and willingness to drive throughout the four-county Central MN area daily
- Fluent in Arabic, Oromo, Spanish, or Somali (preferred)
