Start Today Hennepin is seeking a Housing Case Manager to support individuals in the Supportive Housing program through case management, advocacy, and resource navigation. This role is ideal for professionals dedicated to housing stability, harm reduction, and client-centered care. We operate under a Housing First model, providing immediate access to stable housing without preconditions, followed by individualized supportive services. Housing is treated as a basic human right and serves as the foundation for long‑term stability.
Key Responsibilities Client Support & Case Management- Maintain a caseload of 25–30 clients, meeting weekly to assess needs and support progress.
- Assist clients in accessing mental health, chemical health, medical, and community resources.
- Develop, implement, and monitor individualized goal plans focused on stability and self‑sufficiency.
- Document client progress accurately, including case notes, goal plans, and service coordination.
- Use harm reduction and crisis prevention strategies to support long‑term housing stability.
- Collaborate with the Housing Coordinator on the housing application process.
- Conduct weekly home visits to ensure living spaces are maintained appropriately.
- Maintain positive relationships with landlords and property managers.
- Organize and lead social or recreational community engagement activities.
- Work with the team to develop and implement resident‑focused programs.
- Attend agency meetings, trainings, and partner meetings.
- Adhere to and support the mission and objectives of Start Today Hennepin.
- At least one year of experience working with individuals experiencing homelessness, mental health conditions, and/or chemical dependency.
- Bachelor’s degree in Social Work or related field preferred. Relevant experience may substitute.
- Strong written and verbal communication skills; ability to work independently and as part of a team.
- Proficiency in Microsoft Word, Outlook, and standard office equipment.
- Valid driver’s license, insurance, and reliable transportation.
- Work occurs both in the office and in the field, including frequent travel to client homes. Remote;
Tuesday & Thursday. - Ability to climb stairs, sit, drive, and occasionally lift 15–30 lbs.
