Nurse Practitioner – Geriatric Primary Care; SNF, LTC, ALF

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in skilled nursing, long-term care, and assisted living facilities.

• Perform patient assessments, manage acute and chronic conditions, order diagnostics, and develop treatment plans.
• Prescribe medications, educate patients and families, and collaborate with care teams to ensure seamless care delivery.
• Maintain accurate documentation and support quality improvement efforts.

Qualifications include a Minnesota NP license, board certification, and preferred experience in geriatric or post-acute care.
