Park Nicollet is looking to hire a Registered Nurse - Transition Liaison to join our Homecare team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary:

The Transition Nurse Liaison will be responsible for coordinating the care of patients in Health Partners' enterprise Hospital post-acute care network whose medical condition indicates that they will benefit from Home Care/Infusion Services. This position will assume primary coordination responsibility for Home Care/Infusion referrals to Methodist Home Care (MHC) and/or Park Nicollet Home Infusion (PNHI). The Transitional Nurse Liaison will work closely with, pharmacy, nursing, social services, physicians, rehabilitation services, patients, and their families.

Work Schedule:

M-F 8am-4:30pm

Required Qualifications:

* Education, Experience or Equivalent Combination:

* Minimum of two years, hospital-based acute care nursing experience.

* Two years' experience in a home care setting.

* Licensure/ Registration/ Certification:

* Current Minnesota R.N.

* License BLS provider status in accordance with American Heart Association (AHA) guidelines

* Current MN driver's license

* Knowledge, Skills, and Abilities:

* Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.

* Requires basic computer/keyboarding skills.

Preferred Qualifications:

* Education, Experience or Equivalent Combination:

* BSN preferred.

* Previous hospital-based case management experience strongly desired

* Nursing experience in specialty areas including, but not limited to oncology, neurology, or orthopedics.

* Knowledge, Skills, and Abilities:

* Knowledge of how hospital departments interact and how patients flow through a hospital setting.

* Knowledge of external post-acute and sub-acute health care providers/systems/options in the Twin Cities and greater Minnesota.

* Knowledge of word processing, spreadsheet, and presentation software desired.
