Requirements:
- Current state license as a Registered Nurse
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Homecare Homebase Software Experience
- OASIS Experience
- CHPN certification (Preferred)
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance
Roles & Responsibilities :
- Assess and respond to the needs of patients and families in varied settings.
- Develop, document, and evaluate the Plan of Care (POC).
- Educate patients and families on care, medications, and safety.
- Supervise and provide clinical directions to home health aides.
- Perform on-call responsibilities and provide care as needed.
- Ensure compliance with third party payer requirements.
