Hennepin County Mental Health Center (MHC) is looking to hire two dynamic Staff Nurses with a strong dedication to decreasing healthcare gaps and improving access to care for all individuals. These Staff Nurses will support Hennepin County residents in their efforts to work toward their recovery vision through the use of the nursing process and person-centered practices at both of our Minneapolis clinic locations.

Location and hours:
These positions are located at the Mental Health Centers at the South Minneapolis Service Center at 2215 East Lake Street, Minneapolis 55407, and at the Behavioral Health Center at 1800 Chicago Avenue, Minneapolis, MN 55404. Work hours may vary but will be Monday through Friday, between the hours of 7:30 a.m. and 6:30 p.m. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis.

In these positions, you will:
* Perform face-to-face and phone triage to assess needs and provide care to patients with behavioral health and medical conditions; use clinical nursing judgment to assess patient's compliance with and response to medications, review and assess for side effects, and consult with prescribers to adjust treatment as necessary; educate patients and their families concerning their treatment plan and help them to establish and meet treatment goals in support of their recovery vision.
* Provide care coordination of a panel of clients to ensure holistic needs are being addressed by the interdisciplinary treatment team. This may include communicating with other health care providers, therapists, case managers, schools, congregate facility staff, corrections staff, family members and caregivers, and others involved in a client's care. Participation in weekly interdisciplinary team meetings.
* Assist prescribers in drop-in medication management visits; provide hands on care including injection of long-acting medications; supporting medication assisted therapy treatment, performing and assessing vital signs, DISCUS exams to assess side effects, review allergies, medication reconciliation, and documentation of social histories.
* Provide motivational interviewing, patient education, and nursing assessments/skills as ordered to improve access to primary care services and health screenings in order to support residents with overall wellness and treatment goals.
* Review and document pertinent information in Electronics Health Record (Epic) in accordance with MHC standards.
* Complete medication orders/refills in a timely manner according to MHC established protocols after review of patient record; clarify orders with pharmacies and provide information required for state medication monitoring programs.
Need to have:
* Graduated from an accredited school of nursing.
* Possession of a valid license, in good standing, as a Registered Nurse, or a permit to practice, issued by the State of Minnesota.
* Current CPR certification.
Nice to have:
* Certification as a Public Health Nurse and/or Psychiatric Nurse by the Minnesota Board of Nursing.
* Experience with:
* Providing three or more years of professional nursing in behavioral health or integrated care setting.
* Providing patient education and facilitating health groups.
* Point-Of-Care Testing and Phlebotomy.
* Motivational Interviewing and Person-Centered Planning.
* Working with underserved and/or unhoused populations.
* Working with adults experiencing homelessness and mental illness or co-occurring psychiatric/substance use disorders.
* Working in a primary care clinic, home care, or performing care coordination duties.
* Knowledge of:
* Chronic disease management strategies.
* Mental health diagnosis and psychopharmacology.
* The SAMHSA's 10 x 10 Wellness Campaign.
