General Description:

Under the joint supervision of the Oncology Program Manager and the Care Navigation Manager, the Oncology Nurse Navigator (RN) serves as a consistent care coordinator throughout the continuum of cancer care for patients within the Parrish Healthcare community. This role is designed to guide patients and families through the complexities of cancer diagnosis, treatment, and survivorship by providing education, emotional support, and access to resources.

The Oncology Nurse Navigator functions as a liaison between patients, their families, physicians, and multidisciplinary care teams, ensuring timely care and eliminating barriers to treatment. The Oncology Nurse Navigator is responsible for promoting and demonstrating Parrish Healthcare's Culture of Choice®; timely and accurate processes associated with pre-registration, insurance verification, pre-certification, and insurance notification.

Key Responsibilities:
  • Coordinates and collaborates with Parrish health System entities in the practice, community and the hospital to identify persons appropriate for Oncology Navigation through the use of EMR and analytical systems.
  • Serve as a primary point of contact for oncology patients from diagnosis through survivorship or end-of-life care. Coordinate timely appointments, diagnostic testing, and referrals.
  • Facilitate communication among the multidisciplinary team to ensure seamless care. Advocate for the patient's needs, values, and preferences across the care continuum.
  • Educate patients and caregivers about diagnosis, treatment options, clinical trials, side effect management, and survivorship. Provide culturally competent and individualized education throughout the cancer journey.
  • Offer emotional and psychosocial support to patients and families. Connect patients to social work, financial counseling, and community resources as appropriate.
  • Participate in tumor boards, cancer committee meetings, and quality improvement initiatives.
  • Support adherence to national oncology standards (e.g., Commission on Cancer, NCCN guidelines).
  • Collect and report data for quality measures and navigation outcomes.
  • Engage in community education efforts, cancer screenings, and outreach events aligned with the Commission on Cancer (CoC) Standards.
  • Promote cancer prevention and early detection efforts in underserved populations.
  • Always serves to fulfill the mission; practices and adheres to the Code of Conduct/Standards of Behavior/Values.
  • Demonstrates knowledge and understanding of organizational policies, procedures, and systems.
  • Participates in process improvement initiatives.
  • Performs similar or related duties as assigned.
  • Knows fire, disaster and safety procedures and regulations as it pertains to the work area.
Requirements:
Formal

Education:

  • Bachelor's degree in Nursing required; MSN preferred.
  • Graduate of an RN education program (accredited preferred)
Work Experience:
  • Minimum of 3 years of nursing experience is strongly preferred in oncology, patient navigation, or case management.
  • Familiarity with NCCN guidelines, oncology pathways, and standards of care.
Required Licenses, Certifications, Registrations:
  • State of Florida RN
  • American Heart Association (AHA) Basic Cardiac Life Support (BCLS) Certification required.
  • Oncology Patient Navigator Fundamentals Certificate required within 12 months of hire.
  • National Nurse Navigation Certification: required within 3 years of hire and then maintained per renewal requirements (examples: CBCN, ONN-CG, OPN-CG).
