Nurse Practitioner – Geriatric Primary Care; SNF, LTC, ALF

Overview

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care centers, and Assisted Living Facilities.

• Perform patient assessments, manage acute and chronic conditions, order diagnostics, and develop treatment plans.
• Prescribe medications, educate patients and families, and collaborate with care teams to ensure seamless care.
• Offer autonomy in clinical practice within a supportive team environment, with flexible scheduling and consistent patient panels.

Minnesota NP license, board certification, and 1+ year experience preferred. Strong geriatric and primary care skills required.
