OverviewWe are seeking a dedicated RN Care Manager to provide patient-centered care management services within both the Emergency Center and inpatient settings. The ideal candidate will work collaboratively with the inpatient and ambulatory care teams, patients, and their external support systems to ensure seamless care coordination. This role involves progressing patients towards discharge or the next level of care by identifying barriers and implementing effective solutions.
Responsibilities- Identify and address care coordination needs in collaboration with healthcare teams and patients support systems.
- Facilitate patient progression towards discharge or next level of care by identifying barriers and implementing interventions.
- Work with patients and their external support systems to secure appropriate and safe transition plans.
- Collaborate with physicians to ensure timely discharge and appropriate follow-up care.
- Bachelor s Degree with at least one year of hospital-based med-surg nursing experience.
- Current Minnesota Registered Nurse (RN) license.
- Exceptional organizational and time management skills.
- 1+ years experience in case management, home care setup, and discharge planning within a hospital or nursing home setting.
- Proficiency in EPIC and utilization management.
Skills & Qualifications
- Experience with acute care and care management.
- Strong skills in discharge planning.
This position operates Monday to Friday from 8:00 am to 4:30 pm. The role primarily involves meeting with patients and their families to coordinate treatment and discharge plans. The RN Care Manager typically manages 12-15 patients per day and works closely with physicians to finalize discharge procedures. A two-week training period is provided to learn specific systems and protocols.
Job Type & LocationThis is a Contract position based out of Minneapolis, MN.
Pay and BenefitsThe pay range for this position is $40.00 - $50.00/hr.
