We are seeking a full-time Nurse Practitioner to provide high-quality primary care in Skilled Nursing Facilities, LTC, and ALFs in Minnesota.
Responsibilities- Perform patient assessments, manage acute and chronic conditions, and develop treatment plans.
- Order diagnostics, prescribe medications, and educate patients and families.
- Collaborate with care teams to ensure seamless, patient-centered care.
- Maintain accurate documentation and support quality initiatives.
Licensure: Minnesota NP license, board certification, and preferred 1+ year experience in geriatric or post-acute care.
