Grace Hospice is hiring an experienced hospice Registered Nurse (RN) in the role of a RN Case Manager Hospice.
The ideal RN Case Manager (RNCM) is someone whose dedicated clinical expertise results in terminally ill patients receiving exceptional end‑of‑life care. In this role, you will collaborate with an interdisciplinary team to provide end‑of‑life care to those needing hospice services in their home or community setting. You will provide individualized case management, admission facilitation, clinical support, continuous care, after‑hours call, and/or other skilled nursing expertise with chronic and end‑of‑life care.

Shift Available8:00 am – 5:00 pm, Monday – Friday.
Wage Range$83,000 – $95,000 annual salary.
RN Case Manager Responsibilities- Succinctly convey patient assessments with clinical data to hospice physician who will determine hospice eligibility.
- Provide direct patient care to terminally ill patients; assess, identify and address their individual physical, psychosocial, safety, and comfort needs.
- Anticipate, prevent and treat undesirable symptoms or secondary symptoms.
- Develop, maintain, and manage an individualized care plan that captures the evolving needs of the terminally ill patient.
- Train, instruct and support the patient, family, and caregiver.
- Document patient problems, nursing assessments, patient goals, care provided and patient/family outcomes from interventions and care provided in a timely manner.
- Consult and collaborate respectfully with the interdisciplinary team and others involved in the patient’s care.
- Prepare and present all needed information for bi‑weekly IDG meetings.
- Graduate of accredited school of nursing.
- Current state of Minnesota RN licensure.
- Three years recent nursing experience in acute, long‑term care or home care/hospice/public health.
- Patient care planning and patient teaching skills.
- Critical thinker with excellent written and verbal communication skills.
- Current Minnesota driver’s license, insurance and daily access to reliable vehicle.
