Nurse Practitioner / Physician Assistant - Primary Care
Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and provide exceptional team‑based care to patients residing at Assisted Living and Memory Care communities in and around Minneapolis.
Position Overview- Provide on‑site, team‑oriented primary care to patients within Assisted Living and Memory Care communities
- Full‑time providers care for patients Monday‑Thursday; option for Fridays to be a clinical or an administrative day
- Manage a panel of 200 patients (approximately 12‑16 patients per day with 14 being the average)
- Ability to design and manage your schedule
- Develop and implement care plans appropriate to patients and their desires
- Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
- Access to diagnostics such as labs and x‑rays as needed
- Collaborate with community partners within home care, hospice, and other resources as appropriate
- Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
- Board Certified/Board Prepared through AANP, ANCC, or NCCPA
- Licensed or ability to obtain licensure through the Minnesota Board of Nursing or Board of Medical Practice
- Ability to obtain and /or maintain DEA certification for the State of Minnesota
- Proven ability to communicate effectively with strong verbal skills
- Excellent interpersonal and customer service skills
- Must have a valid and unrestricted driver’s license and minimum state‑required liability auto insurance; local travel is required
- 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
- Base compensation plus quality/productivity: $115k - $150k
