RN Case Manager — North Metro Home Care

St Louis Park, MN (Minneapolis and surrounding suburbs)

Pay: $38.00–$42.00 per visit

• Conduct comprehensive assessments and develop individualized care plans.
• Provide nursing care, medication administration, and health education.
• Monitor and reassess patient needs, adjusting care plans accordingly.
• Collaborate with physicians, families, and support staff.
• Supervise PCA/home health aides and mentor new RN Case Managers.
• Maintain accurate documentation and fulfill certification/recertification requirements.
• Participate in on‑call duties and quality improvement initiatives.

• Active Registered Nurse (RN) License
• Two‑Step TB Test (we provide testing if needed)
• DHS Criminal Background Clearance (we provide screening)
• Valid Driver’s License & Auto Insurance
