A DAY IN THE LIFE OF A COMMUNITY LIVING SPECIALISTIn this position, you will provide person-centered care coordination with older Minnesotans and caregivers over the phone and in-person. You will be providing options counseling, advocacy, resource location assistance and education. You will research home and community-based service options and assist clients as they navigate systems and comply with specific protocols and documentation requirements.
QUALIFICATIONS2 years of experience with case management or care coordination in a long-term care, hospital, or community setting
- Compliance with all state licensure requirements
- Bachelor's degree from an accredited college or university in a human services-related field. Social work and nursing degrees strongly preferred.
- Reliable transportation
- Heart for working with the elderly

WORK SCHEDULE FOR COMMUNITY LIVING SPECIALISTThis position is hybrid, working in the community, at home and in the office. Monday - Friday, 8:00 AM - 4:30 PM.
