Salary: $26-30/hour

Position Summary: The Intake Specialist is one of the first lines of contact for patient referrals to hospice. As such, this person must understand the basics of the Medicare Hospice Benefit and be able to communicate this clearly and patiently to families, caregivers, and other referral sources. The Intake Specialist coordinates multiple schedules via text messaging, phone calls and/or email and arranges changes to scheduling as they occur.

Accurate maintenance of the electronic health record is a fundamental aspect of this position.

Qualifications
  • High School Diploma or GED
  • Business College Degree or 2-4 years office experience
  • Ability to type 65 words per minute
  • Working knowledge of computer hardware and software
  • Ability to read, write, speak and comprehend English
  • Excellent oral and written communication skills
  • Ability to prioritize and organize multiple tasks
  • Ability to work independently
  • Excellent interpersonal skills
  • Ability to respect confidentiality
Job Duties
  • Completes intake/referrals and schedules admissions, consults and assessments and follows up with referral sources pro-actively
  • Accurately enters information into the electronic health record in real time to be available to all on-call staff and other team members
  • Able to scan, fax and email documents as needed
  • Participates in staff in-services/training programs
  • Participates in daily morning intake/referral phone call
