About the Job

Objective/overview The Community‑University Health Care Center (CUHCC) is a community health center in south Minneapolis that provides medical, dental and behavioral health care to 12,000 patients each year. The majority of patients have low incomes and are members of immigrant/refugee and/or BIPOC communities.

CUHCC seeks a 1.0 FTE Community Health Worker (CHW) who will help patients achieve their goals related to diabetes, hypertension, sexual and reproductive health and prenatal/postpartum care. Using their cultural knowledge, language skills, life experiences and training, the CHW will provide health education, self‑management skill‑building assistance and navigation assistance with internal and community resources to address patients' social needs (e.g., food, prescriptions, etc.).

The duration of this position is subject to or contingent on funding sources.

The CHW will work primarily in the clinic and in the community. The hours are Monday‑Friday 8:00 am‑4:30 pm.

Reporting relationships This position reports to the Director of Advancement with some oversight by the Nursing Supervisor. This reporting relationship will change in mid‑2026 to the Integrated Care & Community Health Program Manager.

Essential functions

  • Diabetes and Hypertension Management (50%). Assist patients with blood pressure self‑monitoring: distribute blood pressure cuffs, conduct follow‑up home visits for health education, skill‑building, and connection to resources per the CHW Blood Pressure Self‑Monitoring protocol.
  • Provide health education, skill‑building support, and connections to resources for patients with newly diagnosed diabetes and/or uncontrolled diabetes.
  • Conduct outreach activities in select events and settings to connect community members to care.
  • Prenatal and Postpartum Health (35‑40%). Provide health education and care coordination assistance to help pregnant and postpartum patients to
    1) attend appointments;
    2) develop healthy behaviors; and
    3) navigate internal and external appointments and resources.
  • Sexual and Reproductive Health (10‑15%). Reinforce providers' health education on birth control options and STD prevention and treatment.
  • Assist with appointment reminders and scheduling for missed appointments.
  • Assist with cervical cancer screening results notification and related health education.
  • Co‑deliver program to enhance Latino parents' knowledge and engagement with adolescent sexual health.
  • In each of these areas, the CHW will document patient encounters and outreach calls in the electronic health record system (Ochin Epic) per documentation protocols.

Job duties are subject to change consistent with the job class.

Qualifications

All required qualifications must be included in the application materials.

  • BA/BS degree or a combination of related education and work to equal at least four years.
  • Verbal and written fluency in Spanish and passing score on a language proficiency test.
  • Minnesota Community Health Worker (CHW) certificate and/or minimum 1 year of experience providing outreach and/or health education to individuals.
  • Experience providing outreach and/or health education in at least one of these areas: sexual and reproductive health education, prenatal and postpartum, and/or chronic disease management.
  • Experience working with immigrant, refugee, or underserved populations.
  • Ability to use computers and related software including word processing, spreadsheets.
  • This position requires a negative TB test, and a Hepatitis B vaccination.

Preferred Qualifications

  • Experience providing health education in more than one of these areas: sexual and reproductive health education, prenatal and postpartum, and/or chronic disease management.
  • Experience in a clinical setting (clinic or hospital).
  • Experience working in an electronic health record.
  • Experience providing case management or care coordination services.
