Position Overview: Program Manager
- Waiver Case Management provides person-centered case management services to individuals diagnosed with serious and persistent mental illness, substance use disorders and co-occurring medical conditions who are eligible to receive services through the Community Access for Disability Inclusion (CADI) or Brain Injury (BI) Waiver Case Management services through Ramsey County. The Program Manager will train, advise, and consult with other Waiver & Dual Case Management employees.

• Lead, supervise, and develop the team to support business needs through engagement, motivation, and retention.
• Manage and maintain a caseload of individuals who are diagnosed with serious and persistent mental illness, substance use disorders and co-occurring medical conditions throughout Dakota & Ramsey counties.
• Visit clients at least once a month and twice annually in accordance with waiver-specific rules.
• Coordinate client intake and discharges, within set parameters and timelines.
• Collaborate with other professionals such as physicians, mental health providers, and other teams as needed to develop and update plans for each client.
• Ensure financial authorizations and provider contracts are in place.
• Ensure services are documented in individual records so information is readily available, results of services can be determined, and record-keeping standards are met.
• Access available community and housing resources so that necessary and relevant resources match an individual’s needs and preferences.
• Provide crisis assessment and intervention so individuals receive the assistance they need to be safe in their home and community.

• Must meet any of the three qualifications:
• Be a graduate from an accredited four-year college with a major in social work, psychology, sociology or a closely related field.
• Be a graduate from an accredited four-year college with a major in any field and 2000 hours of experience as a social worker/case manager/care coordinator in a public or private social service agency.
• A public health or registered nurse providing case management must be licensed under Minn. Stat.
• At least two (2) years of management experience.
• A background in Waivered Case Management.
• Knowledge of community resources and how to access them.
• Willingness to travel within the Metro Area, for applicable mileage reimbursements, with a current driver’s license, clean record, and personal vehicle with insurance; and able to transport clients in that vehicle.
• A valid Driver’s License, reliable vehicle, satisfactory driving record, and auto insurance are required.
• Must not be actively receiving services from Residential Transitions Inc. and must not have received services within the last two years.

• Master’s degree in Human Services
• At least one (1) year of direct supervisory experience in Waivered Case Management.
• Knowledge of Procentive software.
• Experience with third-party billing and an electronic health record system.
