Designated Manager – 245D Programs Position Summary

The Designated Manager – 245D Programs provides overall oversight and accountability for one of MNCCS’s 245D-licensed services. This role ensures that services are delivered in a person-centered, rights-respecting, and culturally responsive manner
, while maintaining full compliance with Minnesota Department of Human Services (DHS) licensing requirements.

The Designated Manager partners closely with Designated Coordinators, supervisors, and Direct Support Professionals (DSPs) to strengthen service quality, build staff competency, and ensure individuals receive consistent, meaningful, and safe supports. This position balances regulatory compliance
, staff coaching
, and quality improvement
, and serves as a key leader in incident oversight, corrective action, and continuous program improvement.

Key Responsibilities Person-Centered Program Leadership
  • Ensure services are planned and delivered in alignment with each individual’s assessed needs, preferences, goals, and cultural considerations
  • Promote and protect individual rights in accordance with Minnesota Statutes §245D.04
  • Monitor service quality and satisfaction of individuals served, legal representatives, case managers, and other stakeholders
  • Ensure dignity, choice, privacy, and self-determination are embedded in daily service delivery
Coaching, Supervision & Staff Development
  • Provide coaching, guidance, and performance support to DSPs and front-line supervisors
  • Ensure staff receive effective orientation, training, and ongoing competency development in accordance with §245D.09
  • Support supervisors in building strong, accountable, and person-centered teams
  • Address performance concerns through coaching, corrective action, and progressive discipline as needed
Program Oversight & Regulatory Compliance
  • Ensure programs operate in full compliance with Minnesota Statutes Chapter 245A and 245D, including license terms, conditions, and variances
  • Maintain up-to-date knowledge of DHS regulations, licensing guidance, and best practices
  • Conduct internal reviews and audits of program operations, staff training, and documentation
  • Serve as a point of contact during DHS licensing reviews, inspections, and corrective action processes
Incident & Emergency Oversight
  • Review and oversee incidents, emergencies, and reports to ensure safety, accountability, and learning
  • Ensure incident reporting and follow-up comply with §245D.11 and §245A.65
  • Coordinate corrective actions, retraining, or system improvements resulting from incidents
  • Participate in or support after-hours or on-call response related to incidents or emergencies, as needed
Quality Improvement & Corrective Action
  • Use data, incident trends, audits, and stakeholder feedback to identify service gaps and improvement opportunities
  • Develop, implement, and monitor corrective action plans in response to licensing findings or internal reviews
  • Track outcomes to ensure sustained compliance and service improvement over time
Minimum Qualifications

Candidates must meet one (1) of the following education and experience combinations:

  • Bachelor’s degree in human services, health, education, or a related field, AND at least 1 year of full-time direct care experience
  • Associate’s degree in a related field AND at least 2 years of full-time direct care experience
  • Diploma in a related field AND at least 3 years of full-time direct care experience
Required Experience & Skills
  • Minimum of 3 years of supervisory-level experience in programs serving persons with disabilities or adults aged 65+
  • Strong working knowledge of Minnesota HCBS licensing requirements, including Chapters 245A, 245D, and §245D.09
  • Demonstrated strength in coaching, mentoring, and supporting DSPs and supervisors
  • Ability to balance regulatory compliance with person-centered practice
  • Strong communication and collaboration skills with staff, individuals served, families, case managers, licensors, and healthcare partners
  • Proven ability to lead corrective action, quality improvement initiatives, and culture change
Working Conditions
  • Monday–Friday, primarily in-office, with regular visits to residential and community-based service locations
  • Frequent collaboration with DSPs, supervisors, families, case managers, DHS licensors, and healthcare professionals
  • Occasional evening, weekend, or on-call responsibilities related to incidents or emergencies
