Overview
Nexus Family Healing is looking to hire a Admissions and Transitions Coordinator to join our newest youth crisis and stabilization center set to open in fall 2025 in Hennepin County
.
This new youth crisis and stabilization center is a 13-bed, short-term residential service for Hennepin County youth with complex mental and behavioral health issues, providing treatment and therapeutic supports while triage, assessment, and transition planning take place with families. The service aims to reduce the use of inappropriate care settings such as emergency rooms, police stations, juvenile detention, or unstable home situations.

Pay and Schedule- Full-time Hybrid opportunity. Working in office and from the community.

1800 Chicago Avenue in Minneapolis - Pay Range: $21-23 hourly

Our Admissions and Transitions Coordinator coordinates ongoing therapeutic services and community support necessary to ensure clients can receive stabilization care.
Primary responsibilities- Client Consultation & Treatment Planning:
Assists clients in linking to services, identifying barriers, and connecting with supports prior to discharge. - Provides consultation and support to families, youth, and adults in need of services.
- Works with teams to create stabilization plans, discharge plans, and supported engagement.
- Develops treatment plans and utilizes a variety of treatment approaches with clients.
- Provides assistance to families regarding resources available to the families and youth that will support healthy functioning of the family unit; serves as a resource to families for consultation and crisis intervention; screens direct referrals for needs and assists in triaging individuals.
- Completes intake screening and assessments as necessary.
- Works effectively with outside organizations to implement plans for adults and children entering crisis services.
- Bridges relationships with adults, families, youth, and outside organizations and provides role modeling for advocating needs.
- Provides timely and accurate communication to supervisory and management staff and team members regarding significant events to ensure continuity of care and safety of clients.
- Creates aftercare plans and follow-up.
- Maintains and completes paperwork and documentation required by the organization and governing/accrediting agencies; maintains current files on all individuals enrolled in services.
- Produces informal and formal reports as requested by outside agencies, parents, referral agencies, courts, etc.
- Facilitates discharge planning meetings and develops a discharge plan that meets the individual’s needs.
- Provides timely and accurate communication to supervisory and management staff and team members regarding significant events to ensure continuity of care and safety of clients.
- Communicates with families, legal systems, schools, mental health providers, probation, and social service agencies; works with outside organizations to implement plans for adults and children.
- Bridges relationships with adults, families, youth, and outside organizations and provides role modeling for advocating needs.
- Maintains database of community resources and contacts for outside providers and organizations; collects data on services, linkages and outcomes as identified.
- Completes the aftercare process of discharge planning, follows up on connections to outside services, and conducts satisfaction assessments for providers and individuals receiving services.
