ScheduleThis position works a 32-hour reduced work week
while being paid for 40 hours.
Avivo’s Ending Homelessness division believes safe housing for all is a social justice issue and a human right. The PATH program supports individuals with serious mental illness (SMI) and/or co-occurring substance use disorders through a continuum of support toward housing and positive behavioral health outcomes.
Your RoleAs a Case Manager
, you will provide direct, field-based street outreach, engagement, and case management services to individuals experiencing homelessness. This role emphasizes person‑centered, trauma‑informed, and culturally responsive support aligned with Avivo’s commitment to dignity and connection. You’ll meet participants where they are—streets, encampments, shelters, and other non‑traditional settings—helping them navigate housing and essential services.
- Provide housing-focused outreach and case management for individuals with SMI and/or substance use disorders.
- Connect clients to supportive services, benefits, identification, and housing options.
- Conduct eligibility assessments and enroll clients in PATH services.
- Advocate with community partners and housing systems to remove barriers.
- Maintain accurate documentation in HMIS and electronic health records.
- Participate in clinical consultations and team meetings.
- Promote culturally responsive, trauma-informed care and support Avivo’s Undoing Racism efforts.
- High school diploma/GED with 4 years of experience OR Associate degree with 2 years OR Bachelor’s degree with 1 year.
- Experience working with individuals experiencing homelessness and/or living with SMI/SUD.
- Knowledge of community resources and housing systems.
- Ability to work in outdoor environments and travel locally (valid driver’s license and insurance required).
