Description

Under supervision, the incumbent is responsible for the overall management of the residents’ home, ensuring that programs are implemented properly to meet the individual goals of residents. The incumbent will carry out duties and responsibilities in accordance with the organizations policies and procedures, applicable to federal, state, local standards, guidelines, and regulations as directed by the Program Director.

Requirements Essential Duties Direct Care
  • Interact with residents by listening to their issues, responding to their requests and concerns, sharing ideas and humor, and participating in meals and other activities.
  • Assist residents with daily living skills, meal preparation, self‑care, health care, and maintenance tasks as needed.
  • Support residents in identifying, planning for, and participating in community events and activities.
  • Assist residents in developing and maintaining friendships and family relationships through various means such as correspondence, phone contact and assisting to plan and coordinate social activities.
  • Support residents in purchasing household supplies, personal items, and groceries.
  • Support residents in identifying, securing, and utilizing transportation based on their individual preferences and needs.
  • Implement behavior support plans, intervene with residents in response to challenging behavior, and diffuse crises situations as they arise.
  • Critically evaluate the quality of supports provided to the residents who receive services and continuously strive for improvement.
Facilitating and Supporting
  • Communicate with, consult, and inform county case managers, other support agencies, and support team members in their efforts to identify and support the desires, preferences, issues, concern, and other supports for residents while respecting the rights of residents served.
  • Network with other service agencies to learn new ideas and strategies for supporting residents.
  • Coordinate or assist in the development of new programs and support services.
Program Planning and Monitoring
  • Develop, implement, and monitor resident support plans or assist Direct Support Professional (DSP) in this process.
  • Facilitate person‑centered planning meetings for residents or assist DSP in this planning process.
  • Coordinate the development of new services for people who are new to the program.
  • Coordinate and facilitate annual, quarterly and as needed consumer planning meetings or assist DSP in this process.
  • Develop and monitor the implementation and documentation of individual progress toward personal goals.
  • Complete required charting and documentation and reports regarding progress toward meeting individual goals and outcomes (e.g., daily charts, monthlies, quarterlies).
  • Observe, monitor and provide feedback to DSP regarding the implementation of individual support plans.
  • Supervisors design, implement and monitor behavior support plans, the use of aversive and derivative procedures (Rule 40) and psychotropic medications.
  • Complete necessary paperwork when a resident is discharged from a program.
Personnel/Staffing Management
  • Coordinate personnel management by training new staff, conducting performance reviews, facilitating teamwork and staff meetings, assist in creating job descriptions, delegating tasks and responsibilities, encouraging effective communication, defusing crises and conflicts between DSP, and implementing grievance procedures.
  • Follow up on reports of DSP injury at work and all workers’ compensation related issues.
  • Upon completing performance reviews, make recommendations regarding merit increases.
  • Provide coaching and feedback to staff regarding performance related issues.
  • Respond to staff questions and crises when they are on‑call (e.g., via pager or cell phone).
  • Initiate “debriefing sessions” or discussions with staff following a crisis or incident or accident.
Scheduling and Payroll
  • Develop staff schedules within budgetary limitations, under agency policies and rules, and in response to residents’ needs.
  • Solicit and approve DSP time sheets.
  • Approve DSP leave (e.g., vacation, sick, and personal).
  • Secure DSP to fill‑in when vacancies occur due to illness, resignation, vacation etc.
