Physical Therapy Assistant

Ascend Rehabilitation, LLC is an integrated health and wellness program for seniors seeking to maintain independence or recover from life‑changing events. Highly trained physical, occupational and speech therapists collaborate with fitness professionals to facilitate individualized programs for optimal health outcomes. We are seeking a Physical Therapy Assistant for a full‑time role to work in home health and assisted living facilities in the Central Twin Cities, Saint Louis Park/Golden Valley locations.

Locations:
Central Twin Cities, Saint Louis Park/Golden Valley, providing care locally at rehab agency sites and home care.

Type: $32‑40/hr per two‑week pay period. Shift: Day. Status:
Hourly.

• Provides professional physical therapy services using recognized and approved techniques and appropriate professional judgment under the supervision of a Registered Physical Therapist.
• Is responsible for providing quality patient care.

Graduate of an accredited Physical Therapist Assistant Program certified by the American Physical Therapy Association. Licensed in Minnesota.

• Demonstrated knowledge of state and federal regulations.
• Experience working with seniors in a skilled nursing facility (preferred).
• Experience in both outpatient and inpatient therapy (desired).
• Ability to accurately plan and carry out assignments without direct supervision.
• High level of commitment to compassionate customer service.

Pay range $28.53 - $34.76/hour depending on experience.
