Physical Therapist – Northwoods Rehab

. This position offers a competitive base pay of $40.00/hr to $50.00/hr.
- Bachelor of Science in Physical Therapy from an accredited program.
- National Certification Examination for Registered Physical Therapist.
- Current state licensure or license eligibility.
- Assume responsibility for ongoing continuing education and professional development.
- Exert up to 50 pounds of force occasionally.
- Exert up to 25 pounds of force frequently.
- Exert up to 10 pounds of force constantly.
- Occasional push and pull of up to 25 pounds.
- Standing, walking, bending, kneeling, squatting, and overhead reach metrics as listed.
- Average work day up to 8 hours.
- Employee may be subject to unpredictable and hazardous patient/resident behavior.
- Provide and direct physical therapy services to patients, including assessment, treatment, program planning, documentation, and communication under physician orders.
- Initiate referrals when appropriate and conduct patient screenings at regular intervals to determine need for intervention and/or treatment.
- Evaluate patients to obtain data necessary for treatment planning and implementation, conduct evaluation findings, and communicate results to patients, family, and other team members.
- Establish individual patient schedules, develop treatment plans with short‑ and long‑term goals, and coordinate treatment plans with patient, family, and other team members.
- Implement and conduct treatment as outlined in treatment plans, monitor patient response, and modify treatment as indicated to attain goals in accordance with physician orders.
- Develop appropriate home or community programming, document results of assessment, treatment, follow‑up, and termination of services, and adapt treatment to meet individual needs.
- Supervise and direct Licensed Physical Therapy Assistant, Physical Therapy Aides, Rehabilitation Techs/Aides, students, and volunteers; review quality of services and effectiveness, provide patient, family, and staff education, and coordinate total rehabilitation program.
- Adhere to facility procedures regarding documentation of physical therapy services, attend in‑service meetings, and participate in quality assurance program as required.
