Blue Stone Therapy was founded in 2009 and provides contract physical therapy, occupational therapy, and speech-language pathology across Iowa, Minnesota, Missouri, Nebraska, North Dakota, South Dakota, and Wisconsin. Our focus is on delivering optimal, individualized rehabilitation by highly skilled professional therapists while developing long-term partnerships between staff and communities.

Description

We are seeking a full‑time Physical Therapist to join our team. Applicants must be passionate about enhancing lives and be fully licensed to practice in the state where they will work.

• Provides screenings and evaluations to determine requirements for skilled physical therapy intervention relative to the patient’s prior level of function (gait, balance, mobility, range of motion, strength, pain, home assessments, discharge planning, etc.).
• Develops and implements treatment plans focused on functional and measurable outcomes, adhering to professional standards of practice.
• Monitors patients’ progress and adjusts treatment plans as needed.
• Provides appropriate supervision for physical therapist assistants in compliance with licensure, regulatory, and company requirements.
• Maintains accurate and up‑to‑date patient records.
• Collaborates with, coordinates, and participates in training programs for community staff, patients, families, and rehabilitation staff.
• Participates in continuous quality improvement activities.

• Successful completion of an accredited Physical Therapy program.
• Current valid Physical Therapist license to practice in the appropriate state.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
