Overview

The Physical Therapist is a member of the Rehabilitation Therapy team who evaluates the physical and/or cognitive needs of a customer, establishes a care plan and provides or directs the individualized care of a customer within the specific scope of practice. The desired outcome of care is typically to prevent further impairments, improve function and the ability to participate in activities in order to promote greater independence and reduce the need for further care.

The Physical Therapist educates the customer, their caregivers or family about their role in the care plan and collaborates with an interdisciplinary care team to provide patient-focused service.

• Evaluate patients and establish individualized care plans within the scope of practice.
• Provide or direct the individualized care and educate patients and families about the care plan.
• Collaborate with the Maple Grove Hospital interdisciplinary care team to ensure high-quality patient service.
• Demonstrate the mission, vision, values, and guiding principles of Maple Grove Hospital.

• Education:
  
  Baccalaureate degree from an accredited school of Physical Therapy;
  Master’s or Doctorate preferred.
• Experience:
  
  A minimum of diagnostic group, hospital-based acute care, inpatient rehabilitation, or outpatient neurological experience during training, residency, or fellowship for new graduates. Demonstrates entry-level competency and skills. Advanced skills with low-incidence or high-risk groups and evaluation tools are preferred.
• Communication and interpersonal skills:
  Strong written and oral communication; ability to interact with diverse audiences and handle conflict constructively.
• Other:
  Ability to maintain quality, safety, and infection control standards;
  English proficiency in reading, writing, and speaking.
• Licensure/Certification:
  Licensed as a Physical Therapist to practice in the State of MN; BLS Certification.

  Weekends by department rotation

  Non-Exempt
• Salary Range: $37.49 - $56.23 per hour, based on experience
• Benefit Eligibility:
  Most part-time and all full-time positions are eligible for benefits
