Internal Case Manager:

The Internal Case Manager will be expected to provide direct services and educational classes while coordinating delivery of services with community resources. The goal is to help individuals residing at the Guest House obtain permanent housing and a self-sustaining income. This position works with our Emergency Shelter, Transitional Housing and Veterans programs.

Essential Duties and Responsibilities:
  • Manage a caseload up to 25-30 clients/guests.
  • Gather eligibility documentation for clients designated program.
  • Administer assessments to determine clients’ needs.
  • Develop an Individual Service Plan (ISP) or Housing Stabilization Plan to meet client needs.
  • Positively and respectfully engage with assigned clients to help them to obtain their goals.
  • Utilize community resources that provide services such as job-readiness, literacy and GED, AODA and Mental Health treatment, counseling, benefits, driver license recovery, and housing.
  • Complete the necessary follow-up or actions to assist clients in achieving the goals or meeting their needs.
  • Ensure that all resident records are organized and properly documented as outlined in funding contracts.
  • Accurately enter data in the agency database and Service Point in a timely manner.
  • Facilitate and participate in community meetings and training for clients.
  • Assist in program evaluation and development by preparing reports, billing statements and other outcomes based documents in accordance with funding contracts.
  • Participate in efforts to improve the overall processes and outcomes of all Case Management Programs.
  • Professionally communicate with service providers, clients, co-workers, and community partners.
  • Work collaboratively with staff from other departments at the Guest House and participate on the Comprehensive Service Team.
  • Participate in community events for networking and collaboration efforts.
  • General work hours are Monday – Friday, however some flexibility will be required to work evenings and possible some weekend hours.
  • Join a least one Guest House staff committee.
  • All other duties as assigned.
Qualifications:
  • Bachelor’s degree and 1 year experience working with the homeless preferred OR Associates degree and 1 year experience working with the homeless required
  • City of Milwaukee residency required.
  • Experience working with Veterans preferred.
  • Maintain a valid Wisconsin driver license, reliable transportation, and vehicle insurance in compliance with agency requirements.
  • Ability to speak Spanish preferred.
  • Strong interpersonal communication skills
Training and

Experience:
  • Computer proficiency (Microsoft Word, Excel, Outlook, and Internet).
  • Familiar with issues related to homelessness, mental illnesses and substance abuse.
  • Understanding of motivational interviewing and stages of change principles.
  • Strong organizational, planning, and time management skills.
  • Good decision-making and problem-solving skills.
  • Ability to plan, implement, participate and follow through on projects relating to the position.
  • Ability to think critically and work independently as well as on a team.
  • Ability to communicate clearly, both verbally and in writing.
